Job Summary:


This Registered Nurse (RN) position involves providing skilled home health care to geriatric patients in the Walla Walla, Washington area. The RN will assess patients, administer medications, develop treatment plans, and collaborate with the interdisciplinary team. The role requires excellent communication, strong time management, and proficiency with electronic documentation systems. A minimum of one year of recent skilled home health experience is mandatory.

Location: Walla Walla, Washington, United States

Responsibilities:

- Conducting patient assessments and developing individualized care plans.
- Administering medications and treatments as prescribed.
- Monitoring patient progress and documenting findings accurately and timely using HomeCare Home Health and Oasis software.
- Collaborating with physicians, therapists, and other healthcare professionals.
- Educating patients and their families about their care.
- Managing a caseload of up to six patients daily, maintaining 30 productivity points.
- Maintaining timely communication and documentation.
- Ensuring safe and efficient transportation to patients within a potentially wide radius (up to 50+ miles occasionally).
- Returning provided documentation tools.

Required Skills & Certifications:

- Active Registered Nurse (RN) license in Washington State or a compact state license.
- Minimum of one year of recent skilled home health experience (within the last three years).
- Basic Life Support (BLS) certification.
- Proficiency in electronic health record systems (HomeCare Home Health and Oasis).
- Valid driver's license, car insurance, and reliable personal transportation.

Preferred Skills & Certifications:

- Bachelor's degree in Nursing.
- Experience with geriatric patients.

Special Considerations:

- This is a day shift position with 8-hour shifts.
- No weekend or on-call requirements.
- The employer assists in grouping clients geographically to minimize travel time.
- Occasional travel exceeding 50 miles may be necessary.
