Position Summary On-Site Clinical Supervisor Registered Nurse (RN) shares responsibilities with the Director of Patient Care Services for ABL HEALTH CARE's overall compliance with State and Federal guidelines and standards of nursing practice. The Clinical Supervisor RN shall be available on the premises or immediately accessible by telecommunications during operating hours when patients are receiving services. The RN shall devote sufficient hours to ensure the quality and adequacy of services provided and staff supervision. Qualifications Registered Nurse with at least two (2) years of recent experience (within the last five years) in a home health agency, primary care clinic, or health facility. Background knowledge and expertise in clinical decision-making for the home health patient population to meet patient needs and contribute to Quality Management reviews. Physical Qualifications Ability to speak and hear clearly Willingness to travel to patients' residences Ability to lift and transfer patients using proper body mechanics Responsibilities Provide leadership and case management for patient home health episodes. Coordinate referrals, evaluate high-risk cases, and establish patient care plans. Supervise home health aides and LVNs, ensuring quality care and documentation. Re-evaluate patient plans of care regularly and after significant changes. Document services, collaborate with physicians, and ensure compliance with standards. Manage caseloads, facilitate team communication, and participate in quality improvement activities. Assist with policy development, community education, and staff training.
