🔹 What You’ll Do:

• Deliver on-site, comprehensive care to aging adults
• Manage chronic and acute conditions with autonomy and support
• Collaborate with interdisciplinary teams to build holistic care plans
• Educate residents and families on health and wellness
• Participate in chronic care management initiatives
• Document thoroughly using eClinicalWorks (eCW)

✅ What We’re Looking For:

• Licensed Nurse Practitioner in New York (unrestricted)
• DEA certification (or eligibility)
• Eligible for Medicare/Medicaid reimbursement
• Bonus: 3+ years of clinical experience, especially in geriatrics or LTC

📍 Schedule:

• Full-time and part-time available
• On-site only (travel to assisted living locations required
